Until you receive notification from the Board that your North Carolina speech-language pathology or audiology license has been issued, you will be in violation of the law if you practice in the profession of speech-language pathology or audiology unless your practice is in areas exempt from the statute.


Instructions & Information:

Please choose the appropriate application that fits your licensure needs. Fill it out, and supply the appropriate required documentation. Photocopying of military ID’s is prohibited pursuant to Title 18, US Code Part I, Chapter 33, Section 701.

Pursuant to NCGS 93B-15.1 (k) instead of payment, you will need to submit proof of your military status or your spouse’s military status. Please note, if you or your spouse do not share the same last name you will also need to supply a copy of your marriage certificate as well.

Once licensed, if you or your spouse are not Active-Duty Status, you will be required to pay the renewal fee; unless the Board office receives the appropriate required documentation verifying Active-Duty Status.
