Out-of-State Temporary Applicant Licensure Process

For a permanent license

  1. Setup your online account using the Applicant Portal (Once this is completed you can login and review the status of your application after the application is submitted).
  2. Fill out the Permanent Application (Reciprocity) and take the Open Book Quiz.
  3. Have an official Letter of Verification sent from your state (email to verifications@ncboeslpa.org or mail to  NCBOESLPA, PO Box 16885, Greensboro, NC 27416).
  4. Submit official transcripts for all coursework listed on the application (email to transcripts@ncboeslpa.org or mail to  NCBOESLPA, PO Box 16885, Greensboro, NC 27416).
  5. Complete the NC SEY Report form with your supervisor and submit it to  Denise Brown.
  6. Submit a signed Clinical Clockhour Summary Sheet (commonly from Calipso and Typhon).
  7. Submit a written request for permanent licensure to Denise Brown explaining that you hold a provisional license in another state but would like to obtain a permanent license in North Carolina .
  8. Provide a passing Praxis score to reporting code 7757 or a letter of verification of your ASHA CCCs.
  9. For credit for supervision in another state, contact the Board office for instructions.
  10. If your degree isn’t conferred on your transcripts, submit a signed Program Director’s page.
  11. Application will be reviewed at the monthly board meeting. Check the board meeting dates on the website for more details.

Note to Applicants: The most common reason for delay in the consideration of an application is error in completing the application accurately, providing the required information, and not following the instructions.
