If you are an out-of-state licensee with an expired license and do not hold an active license in another state, follow these steps:
- Setup your online account using the online system (Once this is completed you can login and review the status of your application after the application is submitted).
- Complete the Reciprocal Application, and Open Book Quiz.
- Have an official Letter of Verification sent from your state (email to verifications@ncboeslpa.org or mail to NCBOESLPA, PO Box 16885, Greensboro, NC 27416).
- Submit official transcripts showing your master’s or doctoral degree conferred (email to transcripts@ncboeslpa.org or mail to NCBOESLPA, PO Box 16885, Greensboro, NC 27416).
- Application will be reviewed at the monthly board meeting. Check the board meeting dates on the website for more details.
- If you hold active ASHA CCCs, have verification of your ASHA CCCs sent directly from ASHA to the Board’s office to verifications@ncboeslpa.org or mail to NCBOESLPA, PO Box 16885, Greensboro, NC 27416).
If you hold an active license in another state, follow the process for reciprocal applicants.
Note to Applicants: The most common reason for delay in the consideration of an application is error in completing the application accurately, providing the required information, and not following the instructions. If you are a NC licensee with an expired NC license (and have not left NC or hold an active license in another state), follow these steps:
- If you do not already have an online account, you will first need to set up your online account using the Applicant Portal (Once this is completed you can login and review the status of your application after the application is submitted).
- Fill out the Reciprocal Application and Open Book Quiz.
- Email Denise Brown to confirm whether you’ve been providing services in NC with an expired license.
- If your license has been expired for less than three years, submit proof of 30 hours of Continuing Education (email to Denise Brown).
- If your license has been expired for more than five years, you will need to submit official transcripts showing your master’s or doctoral degree conferred (email to transcripts@ncboeslpa.org or mail to NCBOESLPA, PO Box 16885, Greensboro, NC 27416).
- Applicants will need to submit an official letter of verification for the most recently active license held other than the NC license (email to verifications@ncboeslpa.org or mail to NCBOESLPA, PO Box 16885, Greensboro, NC 27416).
