Out-of-State Temporary Licensees seeking licensure in NC
Do you want a permanent license?
Yes!
- Fill out the Temporary Application and take the Open Book Quiz.
- Have an official Letter of Verification sent from your state to us. It can be emailed to [email protected]
- Submit official transcripts for all coursework listed on the application. These can be emailed to [email protected]
- You and your supervisor need to fill out our SEY Report form and submit it. It can be submitted to Denise Brown at [email protected]
- If your degree isn’t conferred on your transcripts you will need to submit a signed Clinical/Program Director’s page.
- Submit a signed Clinical Clockhour Summary Sheet.
- Submit a written request for permanent licensure in NC to Denise Brown at [email protected]
- Pay just the application fee ($30) and wait to hear what the Board says about your application after the board meeting. Or pay the application fee ($30) and the full unrestricted license fee ($60), and see what the Board says.

- Fill out the Temporary Application and take the Open Book Quiz.
- Have an official Letter of Verification sent from your state to us. It can be emailed to [email protected]
- Submit official transcripts for all coursework listed on the application. These can be emailed to [email protected]slpa.org.
- You and your supervisor need to fill out our SEY Report form and submit it. It can be submitted to Denise Brown at [email protected]
- If your degree isn’t conferred on your transcripts you will need to submit a signed Clinical/Program Director’s page.
- Submit a signed Clinical Clockhour Summary Sheet.
- Make your Praxis score available to us, the reporting code is 7757.

Out-of-State Licensees with an expired License
You will need to submit the following:
- Reciprocal Application and take the Open Book Quiz.
- Have an official Letter of Verification sent from that state to us. It can be emailed to [email protected]
- Provide evidence of 30 hours of Continuing Education.
- Submit official transcripts from the university to us. These can be emailed to [email protected]

***This applies to applicants who do NOT hold an active license in another state*** If you hold an active license in another state please use the Reciprocal Application, and don’t follow these steps!
NC Licensees with an Expired NC License
Fill out and submit the Reciprocal Application and take the Open Book Quiz.
Submit an email to Kate Young Maffeo at [email protected], stating that you have not been providing services in NC with an expired NC license.
Submit evidence of 30 hours of Continuing Education.
Submit official transcripts from the university to us. These can be emailed to [email protected]
